Overview

Riverside Healthcare is seeking a dedicated and compassionate Certified Home Health Aide (CHHA) to provide high-quality personal care to patients in their home environment. Under the instruction and supervision of a registered nurse, the CHHA will support patients with their daily living activities, assist with household tasks, and contribute to the overall well-being of the patient

 

FTE & Shift: 1.0 FTE (Full-time) | Days

Work Location: Location Varies - Primarily Kankakee County

 

Essential Duties

  • Personal Care: Provide assistance with personal care services, including bathing, dressing, grooming, and range-of-motion (ROM) exercises as directed by the RN or Therapist.
  • Household Support: Help with household tasks such as preparing meals, doing laundry, and performing light housekeeping to maintain a clean and safe living environment.
  • Medication Reminders: Remind patients to take their medications as prescribed.
  • Patient Monitoring: Observe and monitor patients' physical and mental condition, reporting any changes to the supervising RN or Therapist.
  • Documentation: Maintain and submit accurate records of the patient's activities and condition according to agency policy.
  • Safety: Ensure a safe environment by following standard precautions and procedures.
  • Independence: Promote and encourage patients progress towards greater independence.
  • Professional Standards: Adhere to professional standards, agency policies, and federal and state regulations.
  • Meetings: Attend unit meetings as required.
  • Continuing Education: Update knowledge by participating in a minimum of 12 hours of education per year.
  • Demonstrates flexibility with assignments within professional scope/duties/licensure.

 

Non-essential Duties

  • Supply Management: Assist with managing and restocking patient care supplies.
  • Team Collaboration: Participate in care planning and team meetings as needed.

Responsibilities

Preferred Experience

  • Previous experience in home health care is desirable but not required.

Required Licensure/Education

  • Certification: State of Illinois Certified Nursing Assistant (CNA) certification.
  • CPR Certification: Current Cardiopulmonary Resuscitation (CPR) certification.
  • Drivers License: Valid Illinois drivers license with personal auto insurance.

Exposure/Sensory Requirements:

Exposure to: 

  • Chemicals: Alcohol, soap, germicidal disinfectants, chemotherapeutic agents 
  • Video Display Terminals: Average 
  • Blood and Body Fluids: Exposure risk is fairly high; universal precautions are employed in the Home care environment. 
  • TB or Airborne Pathogens: Exposure risk exists; universal precautions employed; any indication of the possible existence of TB would trigger increased precautions as well as further testing of the patient.

Sensory requirements (speech, vision, smell, hearing, touch): 

  • Speech: Required to communicate with patient/families/staff/MD's. 
  • Vision: Required to observe patients and their physical assessment, ability to read reports, ability to read machines.
  •  Smell: Ability to smell and make observations regarding the patient's home environment which may be pertinent to their safety. 
  • Hearing: Necessary to communicate with patient and family; needed to hear patient verbalizations. 
  • Touch: Tactile senses necessary in order to provide personal care, bath, shampoo, etc.
